LifeBridge Health Heart Failure Clinic

Jennifer Fisher, MSN, CRNP, a nurse practitioner at LifeBridge Health Heart Failure Clinic in Westminster, has dedicated her career to improving heart failure care in Carroll County.

Fisher helped establish the Heart Failure Clinic at Carroll Hospital Center in January 2024, making it the third such clinic within LifeBridge Health and the only heart failure clinic in Carroll County.

“I have been a part of LifeBridge Health for the past year and helped to establish the Heart Failure Clinic at Carroll Hospital Center,” Fisher stated. “We strive to help each patient understand their diagnosis and provide education on how to manage heart failure at home.”

Fisher earned her Bachelor of Science in nursing from Stevenson University in 2013 with a 3.431 GPA and her Master of Science in nursing with an adult/gerontology acute care nurse practitioner specialization from Walden University in 2020.

Her professional experience includes working from 2015 to 2024 at UPMC Hanover Hospital, where she first served as a cardiovascular diagnostics nurse before advancing to a nurse practitioner role in 2021. At UPMC, Fisher also established the Heart Failure Clinic for UPMC HVI Southern Region.

Fisher’s work extends beyond clinical visits. She participates in multidisciplinary meetings to improve patient care and hospital processes, establishes the nurse navigator role for identifying heart failure patients and providing education, and works with health navigation and pharmacy teams to address social determinants of health.

“I have helped many patients avoid hospitalization by being able to closely communicate with them at the start of a heart failure exacerbation,” Fisher noted.

In one significant case, Fisher helped a frequently hospitalized patient avoid further admissions for about two months, allowing her to remain at home with family before she passed away.

Fisher received the DAISY Award as a registered nurse in February 2020 and served on the Shared Governance Committee and Quality Improvement and Evidence-Based Practice Committee at UPMC Hanover Hospital.

Fisher has been married to her husband, Josh, for 10 years and together they have two children, ages 6 and 2.
